Job Description – Case Manager/Health Educator
Scope of Work:
The Case Manager/Health Educator provides comprehensive case management and Health Education services to participants of the MIP intervention; an intervention for reducing high-risk behaviors for infection and transmission of HIV among the reentry population. Position will require working some time off-site at collaborating agency locations including the Southern Arizona AIDS Foundation. The Case Manager/Health Educator position is a full-time exempt position and requires the ability to work very flexible hours, including evenings, late nights, and weekends.
Functional Responsibilities:
• Recruits members of the target population for project activities.
• Provide a range of client-centered services that link clients with medical care, substance abuse services, housing, transportation, and other services to ensure continuity of care and coordinated access to appropriate levels of health and support services.
• Conducts one-on-one health education and risk reduction sessions regarding HIV infection and prevention, substance abuse, sexually transmitted disease, and safer sex.
• Keeps current on HIV/AIDS, substance abuse, Hepatitis, and other health-related issues.
• Attend and participate in team meetings with providers and network with other agencies.
• Conduct home and/or correctional setting visits to assess and support client.
• Maintain timely and accurate documentation of all client contacts.
• Serve as member of MIP services team and attend agency staff meetings.
• Comply with employment and quality assurance policies and procedures.
• Perform other tasks as assigned.
• Reports to the Program Manager.
Minimum Qualifications:
• Appropriate fingerprint clearance through the Arizona Department of Public Safety.
• Appropriate clearance through the Arizona Department of Corrections.
• Bachelor’s degree in social services, community health education, or related field; and/or two to four years work experience in a human services or non-retail customer service setting.
• Bilingual (English/Spanish).
• Must have reliable vehicle and valid Arizona Drivers License.
Preferred Qualifications:
• Knowledge of HIV/AIDS issues.
• Two years case management experience working with the reentry population and/or substance users.
• Experience working with Tucson human service providers.
• Effective oral and written communication skills.
• Strong organizational skills.
• Excellent interpersonal skills, sensitivity to cultural and personal diversity.
